The rotator cuff is a group of four muscles and their tendons that wrap around the shoulder joint and keep the arm bone seated properly in the socket. Over time, that damage leads to tendinopathy, a degenerative condition where the tendon tissue has broken down and can no longer repair itself efficiently.
Rotator cuff tendinopathy is different from a full rotator cuff tear, though both can be painful and limiting. Tendinopathy means the tendon is degenerated and inflamed but still structurally intact. Partial tears involve actual disruption of the tendon fibers. Both can cause significant pain with overhead movement, reaching, and sleeping on the affected side. Conservative care is typically the right first approach for tendinopathy and many partial tears before surgery is considered.
Sudden severe shoulder pain after a fall, a sharp tearing sensation, or inability to lift the arm may indicate a complete rotator cuff tear, which requires prompt evaluation beyond what conservative care addresses.
